The M/V Derbyshire was an oil/bulk/ore carrier that was lost in Typhoon Orchid off Japan in September 1980.

All 44 lives on board were lost and no trace of the vessel was found until 1994.

A Formal Investigation into the loss was held in 1987, but not until 1997 (when the wreck site was discovered) was there an order to re-open the Formal Investigation.

This report gives the findings of that independent inquiry, which heard a wealth of new technical and specialist evidence from over 30 experts and witnesses.

It should be a document of recommendation for future ship safety requirements on a global basis and is intended to give impetus to new requirements for ship design, construction and operation in order to cut the number of casualties occuring on the world's oceans every year.

The accompanying CD-ROM contains animation sequences to illustrate the report.
